This form is to be completed for initial approval from the Student Transportation Division . Please note that large items such as hockey bags and musical instruments must be secured and separated from passengers. These items must be secured in a fashion to prevent any objects from coming in contact with a passenger during an accident. Stored items cannot restrict accessible or emergency exits. Items are not to be stowed above the seat backs. Please contact your Depot to inquire about luggage securement equipment availability prior to submitting a Field Trip request.
